Consolidated Health Plans, in conjunction with Risk Strategies, offers you an affordable health plan while you attend University of Bridgeport. From this page, you can access important information about your plan with the links provided.

Need an ID Card? CLICK HERE and complete the online form. Your Student No. or Participant ID is your 6 or 7-digit University of Bridgeport school id number with UB in front of it (UB######). When you have completed the form, click on the Request ID Card button. If the website is able to match the information you entered to your CHP enrollment record, then you will be given a link to get a printable online card.

When seeking care, first consult your Student Health Services. Student Health Services can provide many of your basic health care need and can help refer you to the outside physicians to best optimize the benefits of the health plan.

NOTE: The UB Student Health Insurance Plan for 2015-16 Requires Pre-Certification for

ALL INPATIENT HOSPITALIZATIONS. Call 800-633-7867 to Pre-certify your stay.

Your plan includes the PHCS Preferred Provider Organization (PPO). You are not required to use a PHCS provider, however your out-of-pocket costs may be lower when you utilize a PHCS provider. Please note: If you are eligible for Medicare, the open enrollment period for the Medicare Prescription Drug program (Medicare Part D) begins October 15, 2015 and ends December 7, 2015. If you are currently eligible for Medicare or will become eligible after this enrollment period, please contact Consolidated Health Plans for information on your current prescription drug coverage with Nationwide and your options under Medicare’s prescription drug coverage. This information can help you decide whether or not you want to join a Medicare drug plan.
